RN CASE FACILITATOR

Primarily responsible for the coordination and management of inpatient administrative clinical services that includes -- utilization/case management and discharge plan! ning and to ensure that patient’s plan of care and individual needs are met.
Essential Job Responsibilities:
1. Primarily responsible for the coordination and management of inpatient administrative clinical services that include -- utilization/case management and discharge planning.
2. Collaborates with the healthcare team in promoting the provision of efficient, cost effective, high quality hospital and medical staff services.
3. Responsible for assuring patients receive post-discharge services, as appropriate, which extend beyond their hospitalization to assure continuity of services.
4. Collaborate with physicians and other multidisciplinary teams to develop a plan for each patient from admission to discharge.
5. This job requires employees to demonstrate initial and annual competencies in order to remain in the position.
6. Performs other duties as assigned by management.
7. Performs continuous assessments and evaluations to! ensure patient is progressing towards desired outcomes.
8! . Assesses and responds to patient/family needs by coordinating efforts of other team members.
9. Identifies and resolves barriers that hinder effective patient care.
10. Collaborates and consults with physicians on patient's progress and discharge planning needs.
11. Performs utilization review ensuring admissions meet criteria for appropriateness of care and medical necessity.
12. Coordinates the discharge planning process ensuring involvement of all members of the healthcare team. Counsels with patients and family members in decision making and in meeting psycho social needs of the patient.
13. Maintains knowledge of resources and facilities available to patients and family members.
14. Compiles, evaluates, and reports statistics to members of the team and utilizes the information to facilitate process improvement activities.
15. Orients and mentors new staff members.
16. Follows Standard Precautions using personal protective equipment as ! required.
Requirements
Education: BSN required.
Licenses: Active Maryland Nursing or compact state license required.
Experience: Registered Nurse with demonstrated competency in utilization review, and discharge planning functions. Must have strong knowledge base of utilization and insurance requirements, community resources and requirements, and regulatory requirements. Must have strong communication and organizational skills.
Work Schedule:
Exempt Status -- Monday to Friday with some weekends to meet the demands of the department and to support the needs of our patients.
Physical Requirements:
Minimum lifting requirement 50lbs. Primarily sedentary
Performance Requirements: Ability to identify priorities; must be flexible to assure completion of assignment. Exhibit service excellence. Excellent listening and communication skills are required along with the ability to! establish effective working relationships with patients, families, phy! sicians, nurses and all individuals involved in the patient’s care.
