Case Manager- Baltimore City- works with pediatrics and/or adult members who have complex medical co-morbidities. Will be responsible for anticipating and coordinating ! care/services for targeted health population with chronic disease conditions or high cost specialty care. Demonstrates knowledge of case management, prognosis and outcomes. Responsible for ensuring that patient goals are achieved within a specified time period based upon patient activation, individual needs, reasonable expectations and well-documented outcomes. May require travel to hospitals/clinics/doctor’s offices/patient homes. Telecommuter position who reports to the Program Manager, Case Management.

THIS POSITION WILL BE FILLED AS EITHER AN ASSOCIATE CASE MANAGER OR CASE MANAGER DEPENDING ON THE QUALIFICATIONS AND EXPERIENCE OF THE SELECTED CANDIDATE:

Requirements for the Associate Case Manager: - Bachelor's degree from an accredited school of nursing/related field or RN currently enrolled in a Bachelor's program with a documented plan for completion. Candidates with a Master's degree in Social Work are also accepted. - Current RN or LGSW li! censure in the state of Maryland. - A minimum of two (2) years! of clinical practice. - A minimum of one (1) year case management experience.

Requirements for the Case Manager: - Bachelor's degree from an accredited school of nursing/related field or RN currently enrolled in a Bachelor's program with a documented plan for completion. Candidates with a Master's degree in Social Work are also accepted. - Current RN or LCSW-C licensure in the state of Maryland. - Five (5) years of clinical practice. - Three (3) years of case management experience.

Both levels require: - Ability to work independently from a home or clinic office with minimal supervision. - Effective verbal/written communication skills. - The ability to affect change. - Knowledge of computer, general office, and communications equipment. - Knowledge of health care and managed care delivery systems. - Certification in Case Management (CCM) preferred. Community-based acute care setting experience preferred.
