The position of CSLA Counselor works with individuals with disabilities in their living environment to promote independence and personal growth. The incumbent works closely with and under the guidance of the CSLA Coordinator and/or Manager! , Supported Living
ESSENTIAL RESPONSIBILITIES
Work with assigned consumers to assist them with their Activities of Daily Living in a person-center and respectful manner.
Communicate with the other CSLA Counselors, CSLA Coordinator and other interdisciplinary team members on behavioral and independent living skill issues that occur.
Under the guidance of the Nurse Case Manager, administers medications as outlined in the consumer PMOF and in accordance with agency and Maryland Board of Nursing Guidelines.
Provide services as outlined in the person’s individual plan â€" ensuring goals are developed, implemented, and progress benchmarks are being met.
Demonstrate techniques of positive behavioral management and communicate with individuals served in a person-centered, age appropriate manner.
Complete progress notes, daily logs, timesheets and other documentation thoroughly, accurately, and in a timely manner.

Facilitate individual self-esteem through positive intera! ctions, and building a relationship of trust.
Develop community, self-help and independent living activities that follow the person’s individual plan.
Transport individuals for community activities, medical appointments and other individually based needs.
Maintain accurate records for household operations including money logs, daily schedules, communication records and other required documentation.
Assist the individuals in keeping a clean home and maintaining the house and yard in accordance with licensing and agency policies.
Attend and actively participate in required training, family/stakeholder team meetings and supervision meetings.
Other duties as assigned.

None.
SUPERVISION RECEIVED
The incumbent is supervised by the CSLA Coordinator.
PHYSICAL REQUIREMENTS
Flexible schedule will be required due to varying hours. Must be able to stand/wa! lk 50% of work time, sit and/or drive 50%. Must be able to lift, carry, push and/or pull 60 pounds. Must be able to stoop, kneel, crouch, crawl and reach. All of these requirements are necessary in order to supervise and assist individuals with disabilities.
QUALIFICATIONS
BS/BA in special education, therapeutic recreations, psychology or related field preferred. High School diploma required. Past experience working with individuals with disabilities in a community living environment is preferred. Have participated in/hold certifications in and/or ability to pass all required DDA trainings, Medication Administration training, first aid and CPR. Have a clean driving record with no more than 2 points.
This position may require use of your personal vehicle to transport persons-served. All Melwood transportation policies must be followed and liability insurance on your personal vehicle must be in full force and effect while transporting for or on the ! behalf of Melwood. Melwood will reimburse for mileage pursuant to the T! ravel policy.
