General Summary:
The Patient Care Specialist is responsible for the direct and indire! ct care of the geriatric patient over the continuum of care using the Quality Caring Model. Adheres to Standards and Recommended Practices during the direct and indirect care of the patient. Precepts and mentors others by applying the nursing process to plan and manage the care of patients with non-complex and complex biopsychosociospiritual needs. Assists in the development and/or revision of patient care standards and pathways in collaboration with the Director/Educator. Applies research-based findings to advance practice. Develops patient and staff education materials and serves as a consultant for patient and staff education needs. Expertly negotiates the continuum of care plan. Proactively identifies areas of concern and collaborates with the health care team to problem solve and find solutions. Promotes certification and advances in education. Effectively communicates complex and comprehensive information thoroughly and succinctly. Develops and monitors quality improv! ement issues and assures staff team members are competent to p! rovide care for patients. Understands, utilizes and is able to interpret and follow hospital and departmental policies and procedures.

Minimum licensure/certification required (if applicable):
Current licensure by the Maryland State Board of Nursing as a Registered Nurse.
BLS certified.
Additional Life Support training: ACLS
Competency and Certification in PCCN or Geriatric Nurse Certification
Minimum Knowledge, Skills, & Abilities Required:
Graduate of an accredited nursing program; baccalaureate degree in Nursing preferred.
Minimum of 4 years of experience preferred
Advanced computer skills â€" power point, excel, Microsoft word
Reads, writes, speaks and comprehends English language.
Must have visual and auditory acuity.
Working Conditions:
May be required to lift up to 25 pounds as determined by the nature! of the nursing unit.
Must be able to push, pull and lift patients.
Must be able to push, pull, lift and move patient care equipment, i.e., beds/stretchers.
Must be able to bend body downward at the waist and be able to kneel.
Must be able to utilize a computer for periods of time throughout the shift, dependant upon the specific staffing position.
Provides leadership in a charge position when required.
