More details about this job opportunity kindly see the descriptions. The Connections Coordinator manages the enrichment needs and overall operation of our residents in the Connections Pr! ogram. The Connections Coordinator is responsible for managing all resident needs in accordance with the policies and procedures of Sage Senior Living to enhance socialization, maximize daily living skills, expand personal interests, capitalize on resident abilities, and help residents feel safe and with purpose.
Job duties to be performed:
1. Manage and direct the functions of the Connections Program
2. Organize and lead all social/recreational activities appropriate to the physical and emotional capabilities and interests of the Connections residents
3. Schedule/plan all daily activities for the Connections Program
4. Coordinate with Director of Activities to prepare and implement a monthly activities calendar
5. Supervise, train, and assist Connections Assistant/Resident Aides in providing day-to-day care to the residents
6. Coordinate with the Director of Nursing in the ongoing development and implementation of Service Plans based! on ongoing evaluations of resident needs and family input
8. Assist residents with ADLs during the time residents are participating in the Connections Program including meals
9. Respond to resident emergencies
10. Establish and maintain regular communication with resident family members or contacts
11. Document resident activities including but not limited to ADLs on daily basis
12. Report changes in resident condition to the Director of Nursing
13. Ensure compliance and understanding of resident rights
14. Maintain compliance with state-required topics & number of in-service hours. Attend in-service trainings
15. Attend staff meetings
16. Transport residents throughout building
17. Maintain organized and clean work area
18. Maintain and document all food/fridge related items: fridge temperature, expiration dates, etc.
