Conifer Health Solutions is currently hiring for a Telecommuting Nurse Case Manager for Maryland or Virginia.
Ideal c! andidates are able to :
Utilize innovative tools and provide a wide range of medical management services, which include utilization, case and disease management.
Analyze the data provided through the InforMed tools in order to identify gaps in the health of participants and populations, predict participants at risk and improve outcomes.
Work cooperatively with employer groups, physicians, managers, nurses, allied health staff and external agencies.
Qualifications
RN license in a compact state, preferably in MD or VA. At least three (3) years diverse clinical experience as an RN is required. Must be comfortable working with various populations & age groups Five (5) years UM/CM experience if no bachelor degree BSN & CCM / CPHM / UM certification preferred Strong interest/experience in patient education, quality and care coordination Strong aptitude for computers- basic level skills will not be sufficient Spanish speaking is a plus A! bility to attend a week of orientation in Annapolis, Maryland (expenses! paid)
