SUMMARY : The Life Skills Mentor (LSM) works with individuals in their home at Star Community, and is responsible for their daily living needs. The LSM will work as part of a team to provide a safe, enjoyable, and family environment reflective of each individuals unique talents, desires, abilities and interests.
Condit! ions of Employment : Must have the ability to lift, transf! er and implement restraint techniques. This position requires the availability for flexible work schedules. Demonstrated ability to handle multiple tasks and the ability to speak and write proficiently in English is required.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Care
Will assist each individual withactivities of daily living. Will treat individuals with dignity andrespect. Will honor privacy when providing personal care. Will ensure physical, emotional,personal, spiritual, and social needs of the individuals are met. Will maintain a positive homeenvironment. Will provide opportunities for learningand growth. Will honor choices and preferences ofthe individuals.
Communication
Will communicate with individuals according to his/her natural ability. Will communicate information to residential house manager, health care, behavioral support, residential director, human resources, and the day program using appropriate channels. Will demonstrate pr! oper use ofchain-of-command. Will role model positive communication, discourage gossip, honor confidentiality, and promote team spirit. Will advocate for individuals on their behalf and in their best interest.
Health
To ensure the health and well-being of the individuals:
Will administer medications, will follow all instructions from the health care department, and will provide healthy meal choices in line with prescribed diets. Will follow policies and procedures forreporting information to health care nurses and follow-through of documentation as required. Will administer first aid as required.
Administration
Will complete daily paperwork, documentation of medication administration, goal implementation data, communication logs, and other necessary documentation. Will complete time keeping records. Will complete incident reports per policy and procedure. Will attend staff meetings and trainings as required. Will maintain receipts and re! cords of expenditures. Will follow policies and procedures established ! by Star Community as defined in the Employee handbook.
Safety
Will ensure a safe home environment. Will complete emergency preparednessdrills and documentation as required. Will maintain neatness, cleanliness and overall appearance of the home. Will complete facilities maintenancereports, report maintenance concerns to supervisor and to on-call maintenance department in the case of emergency.
Training
Will maintain all requiredcertifications such as but not limited to CMT, CNA, CPR, and First Aid. Will successfully complete initialtraining and annual updates in accordance with DDA, and Assisted Living regulations and Star Community policy.
Will complete other duties as assigned
Qualifications
MINIMUM Qualifications :
Commitment to the Mission and Vision of Star Community Safe driving record; valid drivers license meeting the requirements of Star Community and the insurance company HS diploma or GED preferred

While qualifications for positions vary, minimum qualifications include:
Commitment to the mission and vision of Star Community Valid drivers license which meets the requirements of Star Community and insurance regulations including but not limited to three years of driving experience with a clean driving record No criminal convictions Drug and alcohol free Ability to lift and transfer 50+lbs and be able to stand, sit, kneel without restriction .

Hourly rates are between $9.34 an hour up to $11.50an hour with benefits DOE.
We are ! a 24 hour operation our current open positions would be either 12 hour !
shifts on weekends, must be able to work varied holidays
