POSITION TITLE: Registered Nurse, Neurology Service Case Manager, VA Maryland Health Care System, Baltimore, MD

WORK SCHEDULE: Full-Time

GENERAL OVERVIEW:
At least one position exists for Nurse Case Manager in the Neurology Service assigned to the VA Maryland Health Care System, Baltimore facility.

DUTIES:
The Nurse Case Manager (CM) is a professional nurse who manages a caseload through coordination of care with physicians, nurse practitioners, providers, other health professionals, and hospital systems. Responsibility for coordinating care occurs for each veteran clinic visit and includes follow-up of all provider ordered testing and follow-up telephone contacts with veterans. The CM monitors patient status while the patient receives care within the Neurology Service and communicates and coordinates care with the Interdisciplinary Team to develop treatment plans for these veterans. The CM will work directly with the nurse practitioner and providers to identify patients in need of care coordination. The incumbent assumes coordinating responsibility for all neurology patients ensuring that patient outcomes are achieved within established time frames. The CM is also responsible for screening consults for sleep services by evaluating the veteran and any available data to determine the course of action necessary to most efficiently and effectively treat the veteran's sleep related issues. The case manager will report directly to the Neurology Clinic Nurse Manager.

QUALIFICATIONS REQUIRED:
Active, current, full, and unrestricted nursing license. The successful candidate must have at least a BSN or Associate Degree in Nursing and a bachelorÂ's degree in a related field. A MasterÂ's degree in Nursing or a related field is preferred. Certification as a Case Manager is highly desirable. At least three years current experience in providing direct care to acute medical/surgical patients is required. He/she must have demonstrated the ability to work effectively as a team member, as well as have effective interpersonal, verbal and writing skills. Physical and mental capabilities to satisfactorily perform assigned duties.

Your salary and grade will be determined by the Professional Standards Board (PSB). The PSB is a peer review process. The PSB will review your individual qualifications, education, professional training/internship(s) and professional experience to determine the most appropriate salary and grade based on your individual accomplishments.
