Neighbor Wellness is hiring Family Nurse Practitioners for its NP managed health clinic at Mondawmin ! Mall in Baltimore, MD 21215.This is an opportunity to be instrumental with an exciting new program that provides autonomy for Family NPs to promote patient/community engagement. The successful candidates will be family practice NPs, must be graduates from an accredited school of nursing and an accredited Nurse Practitioner program. The applicant must have a Maryland Advanced Practice Registered Nurse (APRN) Licensure and a provider card in Basic Life Support (BLS). In addition, the applicant must be Medicaid licensed in Maryland with Physician Referral Agreement(s) in place.
We are hiring full and part time experienced Family Licensed Nurse Practitioners.
Responsibilities
All clinic activity and scheduling for providing primary care services across the lifespan including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance
Ensure patient information is accurately, carefully and completely documented in th! e EMR system
Identify opportunities within the community f! or outreach activities
Provide patient education
Perform non-clinic task as required (reporting/auditing/time keeping)
Complete financial responsibilities pertaining to customer payment
Ensure HEDIS standards recordkeeping is followed and up to date
Minimum of 3 years experience working as a Family Nurse Practitioner
Current Maryland Advanced Practice Registered Nurse (APRN) Licensure
Provider Card in Basic Life Support (BLS)
Medicaid licensed in Maryland with physician referral agreement in place
Effective written, verbal and electronic communication skills
Experience Using EMR system
Ability to work without direct supervision and practice autonomously
Able to collaborate with professional colleagues as necessary to provide superior quality care
Great organizational skills and the ability to multi-task
Comfortable using computer (laptop) and software (Microsoft Suite, EMR, etc.)
