The Prince George's County Health Department is seeking qualified applicants to fill two (2) Community Health Nurse I/II positions, grade A-18/21, for the Nurse Monitoring Program within its Health and Wellness Division.
This is an entry level to full performance professional nursing position, whereby the incumbents are required by the State of Maryland Department of Health and Mental Hygiene (DHMH) to monitor elderly and chronically ill individuals who receive home-based community services. Â
Examples of Work Incumbents direct nursing services to individuals utilizing the nursing process; determine nursing diagnosis, develop and implement a care plan, evaluate clients' health outcome, and determine frequency of nurse monitoring visits; teach and demonstrate knowledge of client care procedures and health promotion concepts to clients, families, health care providers, and other individuals; document and maintain medical records and program statistics in accordance with Health Department and nursing standards; confer with health professionals, support planners, family members, and other designated individuals regarding clients' care/needs; complete billing for nurse monitoring services; attend meetings and professional seminars; and perform other tasks and duties as assigned, which may not be specifically listed in the position description; however, are within the general occupational category and responsibility level typically associated with the employee's class of work.
Minimum Qualifications Bachelor's degree in Nursing or a related field. An equivalent combination of education and experience that has resulted in the attainment of a Registered Nurse's License issued by the Maryland state Board of Nursing will be considered.  EACH APPLICANT MUST INCLUDE INFORMATION THAT CLEARLY DEMONSTRATES THE ABOVE QUALIFICATIONS FOR THIS POSITION.

CONDITIONS OF EMPLOYMENT:Â Upon selection, candidate must:
1. Possess and maintain a current valid Registered Nurse License issued by the Maryland State     Â
Board of Nursing. Please provide a copy of your license and indicate license        Â
number; or a license recognized by the Compact State Agreement.
2. Possess and maintain a valid Cardiopulmonary Resuscitation (CPR) Certification at the time     of appointment.
3. Pass a thorough background investigation.
4.  Possess and maintain a valid driver's license. Â
5. Pass a thorough employment physical examination.
DURATION OF ELIGIBILITY:  Candidates will be selected from a temporary register of eligibles, which becomes effective approximately four (4) weeks after the closing date. Once a selection has been made, the register will expire.
ELIGIBILITY TO WORK: Under the Immigration Reform and Control Act of 1986, an employer is required to hire only U.S. citizens and lawfully authorized alien workers. Applicants who are selected for employment will be required to show and verify authorization to work in the United States.
