This job will have the following responsibilities:
Speak with identified members address member concerns and put proper care in place.
Communicate with members are attributed, to improve the transition from another level of care.
Identify, assess, and coordinate health care services based on the members needs and benefits.
Coordinate with Case Manager supervisors to ensure members receive necessary treatment/accommodations.
Ensure that members are properly accommodated to avoid worsening conditions.
Qualifications & Requirements:
Associates Degree in Nursing; BSN preferred
Current RN licensure
4+ years experience as an RN
Oncology, Complex Medical, Trauma Rehabilitation, or FEP experience preferred
Case Management experience preferred
Good computer skills -- Interested and qualified candidates are encouraged to apply!
