The incumbent RN case manager/delegating nurse is the leader of nursing case management of individuals in which the! nurse retains accountability. The RN will exercise and maintain competence in the scope of practice according to specifications in job classification.
DUTIES AND RESPONSIBILITIES:
This list contains task that are typically associated with the job. It is not all-inclusive and may vary from case to and contingent upon other duties assigned by the Director of Nursing. The RN will complete an initial assessment, analyze the data and determine the individualÂ's healthcare needs. Initial assessment should be completed within 48-72 hours of individual being admitted to agency at the individualÂ's location.
The RN Case Manager is responsible for developing a nursing care plan for individuals to address any new/existing medicalconditions.
The RN Case Manager will develop teaching plans for training on medical conditions and care if not available in MTTP and educational instructions.
The RN Case Manager will complete a visit to the individualÂ's resid! ence or location where services are rendered at a minimum of e! very 45 days.
The RN Case Manager is responsible for determining when an individual does not meet the criteria for delegation of being chronic, stable, uncomplicated, routine, and for whom the RN Case Manager has determined the existence of more complex health issues. The delegation nurse must make home visits minimally every 2 weeks to determine if delegation can be done safely.
May assign nursing tasks and supervise the LPN that works in homes assigned to RN. Regular supervisory meetings with LPN will be conducted, documented and should occur minimally every 45 days.
The RN Case Manager delegates nursing task to the UAP/CNA. Delegated nursing tasks include the administration of medications.
The RN Case Manager/Delegating nurse will only delegate those tasks within the Delegated Nursing function Regulations COMAR 10.27.11
Will perform a comprehensive assessment (using assessment tools and any old medical records available) to determine the individualÂ's! stability and chronic condition; whether tasks/functions are uncomplicated and routine with predictable outcomes; whether tasks/functions are performed at high frequency necessary to retain competency and whether the tasks/functions are inherently low risk to the individual prior to delegating any tasks/functions to UAP/Â'CNA. The RN Case Manager will determine if individualÂ's cn perform self taks.
Will determine if the environment is conducive todelegating nursing tasks) by performing a physical exam of setting for safety prior to delegation.
Must define the task to be done; describe what is to be done, communicate the time frame in which to complete the task; develop protocol for task/functions to be delegated, delegate the responsibility to perform the task/function to the selected staffs.
TRAINING:
The RN Case Manager will develop teaching plans for training on medical conditions and care if not available to MTTP and educations instructions! .
Be willing to take ongoing training to provide best possible care! for individuals.
QUALIFICATIONS:
Bachelor of Science Degree
3-5 Years Work Experience
MD/DC License
Ability to retain confidentiality in compliance with HIPAA
Ability to perform the duties and responsibilities described herein
A valid DriverÂ's License, Good Driving Record and 3 or less points
Valid Social Security Card, TB Test, Criminal Background Clearance, Fingerprint BB Pathogen
