The Registered Nurse develops and coordinates wound/ostomy/continence program operations, inpatient care and patient discharge needs.
Responsibilities:
Provides wound assessments and continuity of care for wound patients in collaboration with referring and/or attending physicians and hospital staff.
Works with the primary care physician to plan, implement, and coordinate the patient's discharge n! eeds for comprehensive skin and wound care.
Coordinates re! sources with primary care physician and other healthcare team members to ensure quality services and outcomes.
Reviews cases for quality improvement; formulates, implements, and evaluates plans for improvement in coordination with primary care physician and other nursing personnel.
Education & Experience:
Minimum of 1 year of RN experience required at date of hire
Bachelor of science in nursing required at date of hire
Licenses & Certifications:
Basic Life Support
Registered nurse
