More complete informations about this career opportunity please give attention to these descriptions. Seasons Hospice & Palliative Care is seeking an LPN/LVN to join our team of professionals. We make a difference every day to patients and families coping with end of l! ife illness in creating a center of excellence in hospice care.

Under the direct supervision of a registered nurse, the LPN/LVN provides continuous care services, in-home respite care services, and personal care for the patient and assists with activities of daily living.

RESPONSIBILITIES:
1. Follows the interdisciplinary plan of care. Is alert to the patient's needs and concerns and teaches appropriate self-care techniques to patients and family members.

2. Provides personal care, including: bathing, dressing, grooming, caring for hair, nail care, oral hygiene, skin care, and foot care.

3. Assists patients in turning, getting up in a chair, or ambulation according to the plan of care.

4. Assists with elimination, including: bed pan, urinal, up to the bathroom, routine foley care and enemas according to the plan of care.

5. Assists patients with meals, if necessary.

6. Performs household! services essential to maintaining a safe and comfortable envi! ronment.

7. Performs specialized procedures as an extension of therapeutic services, prepares and performs treatments using aseptic technique, in accordance with the interdisciplinary plan of care.

8. Administers medications.

9. Observes patients when giving care and reports changes in the patient's condition and needs to the Team Director.

10. Completes appropriate documentation in a timely manner.

11. Participates in ongoing education programs.

12. Follows the policy of confidentiality regarding patients and families.

13. Demonstrates a compassionate attitude toward patients and families that is consistent with the hospice philosophy and mission.

14. Attends patient care conferences as requested by the Team Director.

15. Shares in providing 24-hour seven-day-a-week coverage to patients and families

16. Performs other related duties and special projects as assigned.

QUALIF! ICATIONS:
1. Currently Licensed by the state(s) as a Licensed Practical Nurse/Licensed Vocational Nurse.

2. Current CPR certification

3. Minimum 1 year work experience in hospice, home health, geriatrics, or general medical/surgical nursing.

4. Demonstrated commitment to Seasons Hospice’s philosophy of care, values, mission statement and organizational culture.

5. Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance.
